CDA, CCR, CCD

CDA or Clinical Document Architecture is a flexible markup standard developed by HL7. CDA defines a structure of certain medical records as a way to better exchange medical information between healthcare providers and patients. CDA is based on XML(Extensible Markup Language), and uses HL7’s Reference Information Model(RIM) which puts  data in clinical or administrative context and express how pieces of data are connected. CDA takes advantage of coding system such as SNOMED CT and LOINC.

With the HL7 format using XML and RIM, CDA allows EHRs and other health IT systems to process documents.

CCR or Continuity of Care Record is a standard for the creation of electronic summaries of patient health. Its aim to improve the quality of healthcare and to reduce medical errors by making current information readily available to healthcare providers. CCR contains patient information such as patient demographics, allergies. It’s expresses in XML format.

CCR is XML-based patient care record summary standard designed to provide a standard format to support core data sharing between care providers and systems.

CCD or Continuity of Care Document is an electronic document exchange standard for sharing patient medical summary information. Medical summary includes the most commonly needed information about current and past health status in a form that can be shared by all computer applications. CCD is XML-based specification for patient summary data.

CCD Vs CCR

        • ASTM Continuity of Care Record (CCR):  an XML-based standard for the movement of “documents” between clinical applications. Furthermore, it responds to the need to organize and make transportable a set of basic information about a patient’s health care that is accessible to clinicians and patients.

        • Continuity of Care Document (CCD):  the result of a collaborative effort between the Health Level Seven (HL7) and American Society for Testing Materials (ASTM) to “harmonize” the data format between ASTM’s Continuity of Care Record (CCR) and HL7’s Clinical Document Architecture (CDA) specifications.
